Getting started with our Testosterone Replacement Therapy (TRT) program is straightforward and designed to provide you with the best care possible. Here's a step-by-step guide to help you through the process:

1️⃣ Visit Our Website

Explore Our TRT Page: Head over to our Jack Health TRT page and learn more about the program.

Add to Cart: Add the TRT testing to your cart. If you’re new, create an account and complete the payment process. We will then provide you with a link for your initial consultation.

2️⃣ Complete Your Consultation

Medical History: Fill out the online consultation form with details about your medical history and current medications.

Identity Verification: Upload your health card, valid photo ID (Driver’s License, Health Card, or Passport), A recent selfie to verify your identity and your drug card or benefits/insurance information.

3️⃣ Get Your Lab Work Done

Lab Requisition: After your consultation, our nurse practitioner will review it and email you a lab requisition.

Visit a Local Lab: Book your blood work at a nearby lab. Remember to bring your lab requisition and health card. If you don’t have a valid health card, you’ll need to pay for the bloodwork out of pocket.

Lab Results: Once we receive your lab results, our team will review them and proceed with your virtual consultation.

4️⃣ Prescription and Shipping

Prescription Issuance: If you’re eligible, we’ll issue a prescription.

Medication Delivery: Your medication will be shipped directly to your door, free of charge.

5️⃣ Ongoing Assessments

Quarterly Assessments: Every three months, we’ll conduct a reassessment to ensure your levels and overall health are in good shape. This includes new blood tests.

Consultation Fee: A consultation fee will be billed quarterly for these assessments.
